If you are unable to download the paperwork or print it, please contact Dr. Linda Najjar for an alternative means of delivery. Please return the completed paperwork via email or snail mail at least 1 business day before the time of your appointment so that Dr. Linda Najjar can review it ahead of time. It can be sent and received encrypted if requested. Be sure to read the Professional Agreement that includes a Consent to Treatment, Notice of Privacy Practices, and Telehealth Guidelines. Please also include a copy of your insurance card if you are planning to use it to pay for your services. Dr. Linda Najjar will send you a link to the encrypted HIPPA compliant platform for telehealth services ahead of time. Please log on at least 10 minutes before your scheduled appointment.
